Limited Transportation Available:

If you are an established patient with CBN Health Clinic or have an appointment with one of our providers, we can provide non-emergency transport to and from your home.

What Is Needed:

At least 24 hr Advanced Notice is Required Prior to Appointment
Physical Address of Residence
A Valid Phone Number

If you cancelled your appointment or if it was rescheduled, a 24-hour notification is needed so we may extend services to other patients.
